  • Was the person related to a member of the Conference? Please share the name of that person. Is the person lay or clergy? Retired or active? Where do they serve?
  • Services

  • Please include date(s), time(s), location and address where friends and family will gather.
  • Please include date(s), time(s), location and address of where the services will take place.
  • Please include names of spouse and children. Also, include the number of grandchildren and great-grandchildren.
  • Please include name and address or a web link.
  • Who should condolences be sent to? If possible, please include the name and address of the person with the connection to the Indiana Conference. Feel free to also add an email address.
